Office Nurse (LPN)
GENERAL DESCRIPTION OF DUTIES AND RESPONSIBILITIES:
The office nurse works as part of the medical staff and assists the clinical providers in the provision of primary health care. The nurse provides limited direct patient services and performs specifically designated independent procedures.
SPECIFIC DUTIES AND RESPONSIBILITIES:
- Participates in and co-manages the kid-med program.
- Participates in administrative duties to include clinic research or updating grant.
- Co-manages the clinic team and serves as interim clinic coordinator in the absence of the clinical coordinator.
- Meet with management team on a weekly basis.
- Prepare patients for examination including identification of the chief complaint, the taking of vital signs, height, weight, etc., and ensures smooth patient flow throughout the clinic.
- Keeps ill patients comfortable until seen by a physician or NP.
- Assists physicians and family nurse practitioners in healthcare activities, performance of diagnostic and/or therapeutic procedures, and the administration of medications as necessary.
- Assists with stocking the examining rooms and special treatment rooms with needed supplies.
- Performs other necessary duties as required by the Medical staff to meet the goal of providing primary health care services.
- Conducts age-appropriate patient screening for hearing, sight, blood pressure and other simple routine procedures.
- Collects specimens and delivers them to the laboratory.
- Participates in the Diabetic Collaborative Program, other research projects including community projects. Participates in daily performance improvement activities.
- Maintain medication log sheets.
- Attends and participates in all required meetings.
- Participates in daily team huddles as a team-based approach to care.
PERFORMANCE IMPROVEMENT ACTIVITIES:
Participates in the SWLPHC Performance Improvement activities and performs duties in accordance with applicable standards.
REPORTING RELATIONSHIP:
The Clinic Nurse reports to and is supervised by the Clinical Coordinator or designee.
EVALUATION:
The evaluation of work performance is ongoing and will be performed by the Clinical Coordinator or designee.
This job description will serve as a delineation of privileges for the nurse. A skills assessment checklist will be conducted every year, with the annual performance evaluation.
In addition, by signing this job description you are certifying that there is no physical and mental health problems which exist that could affect my ability to perform the requested privileges documented in this job description.
QUALIFICATIONS:
- Certification of practice as a Licensed Practical Nurse or Registered Nurse.
- Certification with sufficient experience to carry out the duties of this position.
- Must be qualified in Basic Life Support.
Medical Assistant
GENERAL DESCRIPTION OF DUTIES AND RESPONSIBLITIES:
The Medical Assistant (MA) functions in a multi-task role. Some of the duties consist of performing clerical duties necessary to prepare patients for visits, to arrange payments, and to make appointments when necessary. Other duties consist of compiling medical records and assisting in obtaining data for tracking and performance improvement activities. The MA works to provide timely and accurate on and off site laboratory services and is responsible for maintaining laboratory equipment, supplies, records, and procedures in accordance with OSHA, CLIA, CQI, and other applicable standards. The MA is also expected to work with the Medical Director and Nursing Department to improve the quality and efficiency of lab services.
SPECIFIC DUTIES AND RESPONSIBILITIES:
Greet patients and answer telephones in a courteous and professional manner, addresses questions and direct calls.
- Ensures patient confidentiality and provides patients with needed information.
- Examines patient medical records for completeness insuring all required information is obtained at time of visit, notes any deficiencies and refers to supervisor for follow-ups; maintain a variety of electronic health records such as: lab and pathology reports, progress notes and patient referrals.
- Prepare patients for examination including identification of the chief complaint, the taking of vital signs, height, weight, etc.
- Assists in updating patient information in the computer.
- Responsible for filing incoming patient correspondence from outside sources after provider review.
- Works with patients and medical health care team regarding test availability, collection of specimens, cost effectiveness of tests, etc.
- Maintains adequate inventory of laboratory supplies, ensuring that all supplies are viable before use in patient testing.
- Performs other necessary duties as required by the Nursing staff or Medical Director to meet the goal of providing primary healthcare services.
- Is familiar and is compliant with JCAHO standards and guidelines as they apply to the community health center.
- Participates and documents patient education, age specific assessments, and compliance with medication/dietary instructions.
- Participates in organization projects including community projects.
- Participates in daily performance improvement activities.
- Assists other staff members in returning patient phone calls.
- Performs other task deemed necessary to meet patient care needs/clinic needs per request of medical and Nursing team.
- Attends and participates in all required meetings.
- Participates in daily team huddles as a team-based approach to care.
REPORTING RELATIONSHIP:
The evaluation of work performance is ongoing and will be performed by the Clinical Coordinator or designee.
QUALIFICATIONS:
- Minimum of High School.
- Ability to use current office machines including computer.
- Certified in Basic Life Support.
- Knowledge base of basic medical protocols, terminology, and commonly used abbreviations.
- Must have some basic knowledge of JCAHO standards.
- Minimum of six (6) months healthcare experience.
- Completion of an approved program of training for Medical Laboratory Technicians and/or Medical Assistant.
