Our mission is to deliver comprehensive quality health care services by improving the accessibility, availability, and acceptability of such services to the people within the service area.

Our vision in partnership with the community, we will create, develop, and promote supportive service to all people who are medically under-served or needy, so that they may experience all the rights, privileges, and responsibilities as members of the community.

Our community partners include but are not limited to: the Louisiana Primary Health Care Association, Chamber of Commerce, the St. Landry-Evangeline Parish United Way, St. Landry Re-entry Coalition, St. Landry Children and Youth Board, and the St. Landry Rotary Club.

We’re about Commitment!

Since 1993, Southwest Louisiana Primary Health Care Center has been providing affordable outpatient medical and dental healthcare services to our community which includes St. Landry Parish, Northern Lafayette Parish, and Evangeline Parish.

We assist the citizens of St. Landry and surrounding parishes in developing their greatest potential and attain a purposeful life through education, family support, and community resources.

Our Health Care Centers are non-profit corporations governed by a Board of Directors.

Southwest Louisiana Primary Health Care Center is Joint Commission Accredited and PCMH (Patient Centered Medical Home) Certified through Joint Commission.  We are an FTCA Deeming Facility.
